The ACFP continues to advocate for and support team-based care, a pillar of the Patient's Medical Home, by facilitating community- and relationship-building. We are pleased to offer a shared community of practice for interested parties to join the conversation on team-based primary care.
The Team-Based Primary Care community is a community of practice of interested individuals who want to learn, share, and advance team-based care in Alberta's health system, including past participants of the Team Primary Care Alberta forum. This community will help continue the conversations and connections made.
