- Chronic Pain and Addiction - previously known as the Collaborative Mentorship Network (CMN) for Chronic Pain, this community houses past and current PCN Pain webinar recordings, more than 200 learning resources for chronic pain, and is our largest community of practice consisting of more that 350 members of family physicians, nurses, nurse practitioners, pharmacists, social workers, patients, citizens, and health leaders.
- Family Medicine and Primary Care Research - if you are a researcher, primary care providers, student, family physician, or simply interested in family medicine and primary care research, we encourage you to join the community and connect with like-minded people.
- Innovation in Family Medicine - our latest community includes past participants of the Family Medicine Innovation Forum and welcome anyone with an innovative and entrepreneurial spirit. This space also features the top innovations profiles featured at the Forum.
- Medical Students - if you're a student interested in family medicine and wanting a space to connect with other students interested in family medicine, we invite you to join.
- Modernizing Alberta's Primary Health Care System (MAPS) - As the MAPS initiative evolves, the ACFP believe it is vital to bring the membership's voice to the table. This community was created to provide a space for members to highlight challenges and solutions pertaining to the ongoing administrative burden in primary care.
- Planetary Health - This collaborative space aims to unite a diverse group of health professionals passionate about exploring and addressing the vital connections between planetary and human well-being.
- Team-based Primary Health Care - The Team-Based Primary Care community is a community of practice of interested individuals who want to learn, share, and advance team-based care in Alberta's health system.
